Yes you could say: “When I was studying in the USA, I always went to the opera every month”
Actually I think it is possible:
“When he was ten, he could count to ten”
It refers to past general ability so it is perfectly possible

Is it possible to use 'but now' with 'since' as regards the contrast between the sense of contunity to now 'since' carries and the break of that continuity 'but now' expresses? For example, 'We have had 3 hospitals since 2000,but now we have 6.'

The thing is you don’t specify when the break happened. It is almost a contradiction, the sentence “We have had three hospitals since 2000” means we have three hospitals now. But then you contradict it by saying you now have six. So it is a strange sentence, the meaning is clear but I would prefer something like: “From 2000 until 2008 we had three hospitals but now we have six.”
However you can use “but now” with “since”; for example in this sentence with present perfect tenses: “It's been awhile since I’ve seen you, but now you've come back again"
